Student Records
Students may request official documents using the forms below. The processing time is 5 business days and the fee is $10 per document. No refunds will be issued for cancellations, and an email will be sent once the request is completed.
HTIC official academic transcripts and documents are the student’s property and are released only at the student’s authorization. Any document requests from third parties, including family members, must first receive authorization from the student. Then the student must inform the Office of Student Services (OSS) of the authorization through email or writing. Requests submitted by third parties can request a document only to themselves and not to other third parties.

Students may request one official document to send to him/himself or one recipient, such as a:
Transcript
Certificate of Expected Graduation
Certificate of Graduation
Verification of Enrollment
Verification of Past Enrollment
Note: Transcripts that are requested to be sent through email will be sent as an E-Transcript for security purposes. Please check that the institution or organization you plan to email to is able to accept an E-Transcript and/or document PDF file. We do not send E-Transcripts and PDF file documents through third parties such as Parchment and National Student Clearinghouse.
Multiple Documents Request Form
If you would like to send multiple documents or copies of one document to yourself or one recipient, you may do so by using the bottom forms according to the amount you would like to request.
Example: Requesting 1 Transcript and 1 Certificate of Expected Graduation, submit the Multiple Documents Request Form - 2 Documents; Requesting 3 Transcripts, submit the Multiple Documents Request Form - 3 Documents; etc.
